The initial intake form is completed before your first consultation and helps us gather important background information about your child. It includes questions about developmental history, medical background, and current communication concerns. This allows us to come to your consultation prepared and make the most of our time together.


Participate in an initial consultation

The initial consultation is a brief, no-pressure conversation to learn more about your child and your concerns. We’ll discuss developmental history, current communication skills, and whether speech therapy or an evaluation may be appropriate. This is also a chance for you to ask questions and determine if our services feel like the right fit for your family.


Complete an initial evaluation

The initial evaluation is an opportunity to get a comprehensive picture of your child’s areas of strength and areas for growth. The evaluation is typically completed over 1-2 days, depending upon your child’s needs. Following the formal evaluation, a written report is provided and a meeting is held with the parent to discuss results and develop a plan for next steps.


Begin therapy services

If therapy services are recommended, your child will then begin meeting regularly with a licensed SLP for therapy sessions tailored to his or her unique needs and goals.
